I fratelli Winchester giungono nel Wisconsin perché i bambini di una piccola cittadina stanno cadendo in coma ininterrottamente e non si risvegliano. Anche il fratellino del piccolo Michael è caduto preda di questa sorta di malattia e Sam e Dean decidono di aiutarlo perché l’affezione che hanno l’uno per l’altro è molto simile a quella di Michael per il fratello in stato comatoso. Dopo aver appreso dal dottor Hydecker le dinamiche dell’epidemia, Dean comincia a ricordarsi di un grosso errore commesso quando era piccolo e che poteva costare la vita di Sam. In realtà a causare il coma è una Sthraiga, ovvero una strega albanese che si ciba della linfa vitale dei bambini. Per poter riparare allo sbaglio compiuto ai danni del fratello durante l’infanzia, Dean cerca la collaborazione di Michael per poter attirare la Sthraiga ed ucciderla, mentre Sam durante le sue indagini giunge ad una scoperta che rivela la verità, che coinvolge perfino il dottor Hydecker.

John: That’s my man.
Michael: King or two queens?
Dean: Two queens.
Michael: Yeah, I’ll bet.
Dean: Yeah, well. First of all, I’m not going to open fire in a friggin’ pediatrics ward.
Sam: Good call.
Dean: Second, it wouldn’t have done any good cause the bastard’s bulletproof unless he’s chowing down on something. And third, I wasn’t packing. Which is probably a really good thing cause I probably would have just burned a clip in him off of the principal alone.
Dean: I’d give anything not to tell you this, but sometimes nightmares are real.
Sam: I wish I could have that kind of innocence.
Dean: If it means anything, sometimes I wish you could, too.
